What is your current occupation and how long have you been with your current employer?

Why have you considered adding a Maine Coon to your family? Why do you feel they will be a good fit for you & your lifestyle?

Will your kitten be kept indoors, outdoors or both?

Have you ever had a cat declawed? Do you plan on declawing your kitten if approved?

Is anyone in your home allergic to cats?

Are you aware that Maine Coons need regular brushing/grooming? Is this something you are okay with doing for the entire life of your cat?

Do you own your home or rent? If renting, are you allowed to have cats?

Are you an established client at your veterinarians office?

Who will care for your pet(s) when you travel?

Please list the pet(s) that currently reside in your home.

Have you ever surrendered an animal to a shelter or rescue? If so, please explain why and when this took place.

Are you willing to have your kitten spayed/nutered and agree he/she will not be bred?

Are you wanting to purchase breeding rights with your kitten? If yes, please describe your previous experience breeding cats.
